The Wellness Snapshot asks about everyday behavior and circumstance only, and its answers are processed in your browser. Please do not enter health details into any form on this website.

Nothing on this website is intended to make it a HIPAA-covered system, and we do not hold ourselves out as HIPAA-compliant for these pages.

Information you give us: name, email address, phone number, organization details and the content of messages you send through our forms.

Information collected automatically: pages viewed, referring source, campaign parameters, approximate location derived from IP address, device and browser type.

To respond to you, to run and improve our programming, to measure whether the site is useful, and — only where you have separately agreed — to send you marketing.

We do not sell your personal information, and we do not share it with advertisers for their own purposes.
